The App Manager will not show anything in ROM, mate. It is never going to show up there. The app manager will only keep track of the apps installed to User Content Areas (C:\ E:\ and F:\).
i can only say as far as the menu goes, i mod mine via C:\Private\200113DD\content\matrixmenudata.xml
