You need Carbide.ui Theme Edition (http://www.developer.nokia.com/Resources/Tools_and_downloads/Other/Carbide.ui/) to create themes..

It's better to use coreldraw or a svg drawer photoshop is completely the wrong program to make icons.

There's an option in carbide somewhere under 3rd party icons try pressing one of the menus at the top and going into options.
